Hormonal acne in the week preceding menstruation is a manifestation characterized by breakouts influenced by hormonal fluctuations inherent in the menstrual cycle. This type of acne, commonly known as premenstrual or menstrual acne, typically emerges due to the rise and fall of estrogen and progesterone levels. Women experiencing this phenomenon often notice acne concentrated on the lower face, particularly along the chin and jawline. The lesions manifest as inflamed papules, pustules, or occasionally more severe nodules and tend to be more painful and red than at other times of the month.
Cyclical Nature of Premenstrual Acne

Patient Profile
This acne case report features a woman named Sarah, a 32-year-old woman navigating the challenges of motherhood while working in the high-tech industry. As a mother of two energetic young children, Sarah’s days are filled with the joyful chaos of parenting. Juggling the demands of her family with a career in the high-tech field, Sarah found herself grappling with unexpected, persistent acne. Despite her professional expertise, the stressors of her role in the tech world and the demands of motherhood took a toll on her skin.
Troubled by hormonal acne flares, especially in the week leading up to her menstrual periods, Sarah sought solutions that would align with her active lifestyle. Eager to balance her family and professional life, she explored acupuncture to regain control of her skin health and overall well-being.
She presented with a history of irregular menstrual cycles ranging from 28 to 45 days, severe menstrual pain (dysmenorrhea), and persistent hormonal acne flares predominantly on her chin and jawline. In addition, she had night sweats and struggled with irritability and other PMS symptoms the week before her period. Conventional treatments, including topical retinoids, hormonal contraceptives, and an IUD, provided limited relief and were accompanied by unwanted side effects.
Therapeutic Intervention
Sarah explored acupuncture as an alternative approach and came to Raleigh Acupuncture. We conducted a thorough assessment, considering her medical history, lifestyle, and specific symptoms. Her tongue was red with a thick yellow coating reflecting dampness and internal heat. Those are both tell-tale signs of acne. In addition, Sarah’s pulse was wiry, indicating liver qi stagnation, which is a primary cause of PMS symptoms and painful dysmenorrhea. Her treatment plan consisted of weekly acupuncture sessions to clear heat, transform dampness, and smooth the liver qi. In addition, we provided dietary recommendations and stress management techniques.
Acupuncture Sessions
The acupuncture sessions in this acne case report focused on restoring balance by stimulating specific acupoints associated with hormonal balance and skin health. The practitioner selected the following points to regulate Qi flow and address underlying imbalances: LV3, LI4, ST36, SP6, SP9, LI11, and CV12. Additionally, local points around the chin and jawline were targeted to promote blood circulation and reduce inflammation in the affected areas.
Dietary Recommendations
The patient received guidance on avoiding hot and spicy foods, as they would add more heat to her body, which might aggravate the acne. In addition, we counseled her to prepare warm, cooked foods instead of cold, raw choices, as they would strengthen her digestive system and help resolve her internal heat and dampness.
Stress Management
The primary treatment to reduce her stress included acupoints LV3 and LI4 (the four gates), which prompt the liver to keep the qi flowing in the body, especially during her PMS week when she was more susceptible to hormonal acne. In addition, we recommended mind-body techniques, including deep breathing exercises and mindfulness meditation, to help Sarah manage stress levels. Stress reduction was crucial in addressing hormonal imbalances contributing to acne flares.
Acne Case Report Outcomes
After six weeks of acupuncture sessions, Sarah reported a significant reduction in hormonal acne severity. Her menstrual cycles became more regular, stabilizing at an average of 30 days. Most significantly, she experienced a significant decrease in menstrual pain, with pain intensity decreasing from severe to mild.
She continued with monthly maintenance acupuncture sessions and adhered to the recommended lifestyle changes. Over the following three months, her hormonal acne remained well-controlled, menstrual cycles remained regular, and dysmenorrhea continued to improve.
Conclusion
This acne case report demonstrates that acupuncture and lifestyle modifications can be a valuable treatment for managing hormonal acne, regulating menstrual cycles, and alleviating menstrual pain.
